If your fish is fat with eggs, it may be ready to spawn soon!!!!!!!!!!!Smile

This protruded objest in the anus may very well be the egg sac...........

Hummmm.......not pregnant........ it might be a bacterial infection....Mad

First thing, try to give the fish a salt bath (0.3% salt) as they will usually recover very soon.

About 10 minutes of balt everyday, until you see improvements.

Second and most direct route is to apply antibiotics, but since we are not sure what it is, salt baths would do.Tongue

dear vv
it looks sore, the thing is when you see the size of a koi's mouth and then see the size of a koi's bum you wonder why oh why do they sometimes swallow small rocks and stuff. Fish do suffer from constpation. You might have seen them in aquariums with long poo's hanging from them. this is down to bad diet and can kill them. Also overfeeding becouse koi will eat all you give them what they cant digest just gose through.
