Bacterial infection re-oocurence?

Hello everyone. Last fall my favorite koi had a bacterial infection of some sort that caused her scales to become raised near the rear end of her body. Through 3 shots of baytril, salting the pond to .3%, heating the pond to 68-70 degrees " for three weeks before it got too cold", fixing a KH/PH problem we were able to get her looking good again. Recently I have began to notice what seems to be more scales lifting off her body in a different location, " a little closer to her mid-section & only on one side, an even area not like a tumor" I am in Sacramento, California. For the last 3 months I have allowed the rain to dilute the salt level to .12 % and the water temp has not gone below 44 degrees nor over 54 degrees. Feeding them very rarely and very little when temp is up to 50 degrees. Noticed the KH had dropped to 2 again, but the PH was still at 7.5. Since October when my KHA advised me of the low KH I have not been able to get it above 4 but have been able to maintain the PH at 7.5 to 8.0. Haven't checked the Nitrates due to cold water, but ammonia and nitrites at 0. I am starting to raise the salt level again and am thinking that this time I should leave at .3 % until the water warms up in the 70's for 6 weeks to make sure that her immune system has a chance to fully recover. Does this sound like a fair assessment?? Are there any other suggestions?? She is 3 year old Kamabuko, about 18 inches long. Still acting normal as with all the others. Starting to become more active as the weather is warming and sun is shining. I do still have some medicated food, but she wouldn't eat that last fall although the other fish thought it was great.
