Strange gill problem

Hi, I am new to this forum, please help, Our 7yr old koi has an unusual problem. It started as a standard problem of isolating and staying near the top of the water. Water conditions are very good: ammonia, nitrite, nitrate, pH all good. We treated with an antibacterial (with frequent partial water changes) and when that didn't work, eventually switched to an antifungal treatment (not at the same time). During the course of this treatment, his veins became very red and enlarged in his skin and he refused to eat. Then it became hard for him to close his mouth all the way (although it isn't an obvious swelling of the gills) and strings of mucous were visible. Still all of this seems like symptoms I've read before and wasn't worried until he spit out what appears to be an internal structure related to the gills. This is not the red inner part of the gill, but something even more internal? It was white, about 1 inch long with ribbing that is reminiscent of some type of internal filter. Fish is not doing well and will probably die very soon. I have no idea anymore what's wrong. Does anyone know what he spit out? What should I do for the other fish so they don't have a problem? Thank you in advance for any help or advice. P.S. I don't think it's flukes based on other fish, but I don't have a microscope to rule out for sure.
